Infant Plan (Ages 0–3)

- Cavity risk assessment & oral hygiene counseling
- Growth & development monitoring
Child Plan (Ages 4–12)

Includes:
- 2 routine exams
- 2 professional cleanings
- All necessary X-rays related to preventive visits
- 2 fluoride treatments
- 1 emergency exam per year
- Preventive sealant evaluation
- Cavity risk & gingival health assessment
- Oral hygiene & dietary counseling
Teen Plan (Ages 13–17)
Designed for teens transitioning between pediatric and general dental care.
$595 / year
Includes:
- 2 exams
- 2 cleanings (teen prophy)
- All necessary X-rays related to preventive visits
- 2 fluoride treatments
- 1 emergency exam per year
- Monitoring of eruption, spacing, and orthodontic needs
- Oral hygiene coaching tailored to teens
Member Savings
All members receive 15% off the following services:
- Restorative treatment: fillings, space maintainers
- Extractions
- Stainless steel crowns or zirconia crowns
- Pulpotomies
- Frenectomy
- Nitrous oxide inhalation sedation (if needed)
- Behavior management
Exclusions
- Hospital dentistry / general anesthesia
- Specialist referrals
- Cosmetic services (optional depending on your preference)
What’s NOT included
To maintain clarity and prevent misunderstandings:
- Membership is not dental insurance
- Cannot be combined with insurance or other discounts
- Benefits do not roll over if unused
- Additional emergency exams are discounted, but not free
- Membership fee is non-refundable
- Membership covers the patient listed only (not transferable)
